Services: The care provided by Hospice of Guernsey, Inc. is a family-centered approach that includes a team of doctors, nurses, home health aides, social workers, chaplain, counselors, therapists and trained volunteers. They work together to focus on the dying person's needs - physical, emotional and spiritual. The hospice team develops a care plan that meets each person's individual need for pain management and symptom control. Our team is made up of: * Medical Director * Registered nurses * Palliative Pharmacist * Certified Home Health Aides * Social Workers * Chaplain * Counselors * Art Therapist * Reflexologist * Music Practitioner * Trained Volunteers * The personal physician of the patient The medical director's role is to act as advisor for medical services. The staff of registered nurses make regularly scheduled visits to all patients. A registered nurse is on-call 24 hours a day, 7 days a week. The Palliative Pharmacist reviews each patient's diagnosis, symptoms and current medications. Suggestions are made by the Pharmacist for additional medications that would work well to control pain or other symptoms. Certified Home Health Aides help with personal care and light housekeeping. Licensed social workers provide social and emotional support as well as financial counseling and connecting patients with community resources. The Chaplain offers spiritual support for the patient and family. He deals with end-of-life questions from a spiritual perspective. Counselors are available for bereavement support. Art Therapy, Reflexology and Music are available to aid our patients in relaxation and reminiscing. Dietary counseling, physical therapy, speech therapy and occupational therapy are available as needed. Trained volunteers assist patients with identified needs. The bereavement unit is made up of professionals and volunteers who provide assistance from the time of admission until at least one year after the patient dies. Other services provided are payment for medications, supplies and equipment related to the terminal illness, and short-term inpatient care for pain and symptom management or for family respite. |
