Referral Process: Hospice referrals come from several sources. They come from physicians, from patients, from family or friends, from hospitals or from long-term care facilities. Once a referral is received, Hospice of Guernsey, Inc. assists the patient and family in the decision-making process by setting up a meeting to provide information regarding hospice services and the criteria necessary to receive those services. This "Informational" meeting is held in a variety of places. It can be held in the patient's home, in the home of a family member, at the hospital, in long-term care facilities or at our offices. If you think that you, a family member, or a friend may be in need of hospice services, please feel free to contact us and schedule an "Informational". Or, if you prefer, you may request that printed information be mailed to you. |
